DEFANOTHERAPY - New Technique of treatment of vertebral pathology METHODICAL RECOMMENDATIONS N 96/236.
TESTIMONIAL of the method for manual therapy developed by Doctor A.I.Bobyr and protected by Patent No.2,005,456 of Russian Federation entitled "Method for treatment of spondylosteochondrosis and prevention of vertebrogenic diseases"
A total of 210 patients were examined and treated at the chair of vertebral neurology of the Kazan State Medical Academy using the program of the candidate thesis submitted by Doctor A.I.Bobyr and entitled "Clinical aspects and treatment of the fibroid fixed hypomotility in patientds with lumbar ostechondrosis syndrome".
The work was aimed at elaborating objective diagnostic indicants characteristic of the stages of the motor stereotype formation, i.e., the polyregional, regional, and intraregional, which aim was successfully attained. The program of work covered also comparing the efficacy of diverse methods of physical action on the osteochndrosis-affected vertebromotor segment; therefore all the patients were subdivided into two groups depending on the kind of manual therapy performed, that is, the first group included a total of 110 patients who were given manual therapy and psychophysical exercises according to the method developed by Doctor Bobyr; the second group consisted of 100 patients who were subjected to manual therapy according to the classical method. It was the therapeutic procedures elaborated by Doctor Bobyr that proved to be most efficient. An improvement was observed in a majority of treated patients within the first month of the treatment course, and towards the end of the third month of treatment the percent of cured patients amounted to 85, whereas the clinically manifested changes towards convalescence were observed in the first month of treatment in 18% of the patients treated according to the classical method, the percent of such patients being 48 towards the end of the third month of treatment. The patients selected for the comparison treatment exhibited diverse neurological syndromes, that is, lumbago, lumbago-ischiatic, and radicular.
The manual therapy according to the Bobyr's method was found to be most efficient in patients with the lumbago syndrome, somewhat less efficient in patients with the radicular syndrome, and the least efficient in patients with lumbago-ischiatic syndrome.
None of the 110 patients treated by the Bobyr's method developed any complications, which evidences that the method is atraumatic.
In view of the foregoing, we deem it possible to regard the method of manual therapy elaborated by Doctor A.I.Bobyr as an atraumatic highly efficient method of manual therapy which is much simpler to carry out compared with the classical method (Levit's). The method may be recommended for practical mastering to a broad circle of specialists.

REPORT of Clinical Trials of "Method for treatment of spondylosteochondrosis and prevention of vertebrogenic diseases" by Doctor A.I.Bobyr, as per Patent RU 2,005,456
The chair of reflexotherapy of the Russian Medical Academy of Postgraduate Training has trialled the method of Doctor A.I.Bobyr for efficiency on a total of 230 patients suffering from osteochondrosis of the spinal column.
As to the clinical coursing of the diseases the patients were classified as follows: radicular syndrome, 92; lumbago-ischiatic syndrome, 78; and lumbago syndrome, 60 patients. The patients were subdivided into two groups, the first group consisted of 115 patients who were given manual therapy according to the conventional method, and the second group of patients who were given manual therapy according to the method developed by Doctor Bobyr.
The manipulation technique was as follows. The place of action on the vertebromotor segment was determined either classically by the degree of mobility in the aforementioned segment or by a rapid method by detecting a tonic or fibrous induration (i.e., local muscular defense) of the paravertebral muscles which corresponds to the level of the blocked vertebromotor segment. Then, depending on the site of location of the revealed loca}. defense, there was fixed the patient's head (when the local defense was detected in the cervical spine), or a spine portion (when the local defense was detected in other spine portions) proximally to the muscle affected by the local defense, using the thumb or an elastic rod, respectively. Next there were performed once-through jerky tractions in the cranial direction and a pressure was applied to the local defense in the sagittal plane with forces P and P1 stated in the specification of the patent. The tractions were made only with the patient in a relaxed state.
The number of the manual therapy sessions and their repetition frequency depended on the clinical aspects of the disease.
With a view to retaining the result obtained, as well as to strengthening the spinal muscular corset, the treatment was supplemented with a complex of autopsychophysical exercises in combination with breathing exercises as stated in the specification of the patent.
The results of clinical trials demonstrated that after thefirst month of treatment of patients by the method proposed by the inventor the clinical signs of the disease disappeared in 78 patients (68%), in two month after beginning of the treatment the number of such patients amounted to 100 (87%), and by the end of the third month the number of such patients became 105 (91%), while in other 10 patients (9%) the treatment was ineffective. As to the patients treated in accordance with the conventional generally adopted method, an improvement was observed in 40 patients (35%) after the first month of treatment, in 57 patients (50%) after the second month, and in 69 patients (60%) after the third month of treatment. No positive results were observed in the remaining 46 patients (40%).
In view of the foregoing, one may infer that:
- The proposed method for treatment of osteochondrosis and prevention of vertebrogenic diseases is efficient against lumbago-ischiatic, radicular, and lumbago syndromes of osteochondrosis;
- The method is, simple, atraumatic, and can readily be mastered by physicians, whereby it may be recommended for use in preventive medicine and for implementation in widespread medical practice.
